If you are not satisfied with STAT! 5-caps or STAT! Royal Flush, STAT! will refund 200% of the suggested retail price. Refunds on STAT! shampoo are 100% of the suggested retail price.

All correspondence must be sent via certified US mail, postmarked within 30 days of date of purchase. To receive your refund, send ALL of the following:

1. Photocopy of your driver.s license or other picture ID.
2. The UPC code.or the entire bottle.
3. Original sales receipt showing product purchased, date of purchase, name of retailer, and     store location. If you purchased via the STAT! website, please include a copy of the invoice     shipped with your order.
4. Letter detailing your dissatisfaction with the product.
5. Phone number with area code where you can be reached during office hours. A rep may call     you to discuss your results and/or make suggestions for future success.
